A little product, some blending, one last look in the mirror and your eyes instantly appear more rested. It works until eleven o’clock, until you catch yourself under the unforgiving lighting of an elevator or on a video call, and until someone asks the familiar question.

“Didn’t you sleep well?”

Except you did. For eight hours.

And that is where the biggest misconception about dark circles begins. We associate them so closely with tiredness that almost every shadow beneath the eyes is treated as the same problem with the same cause.

In reality, the brown discoloration that has been there since adolescence, the bluish tone that becomes more noticeable after a sleepless night, the shadow created by a deep tear trough and the puffiness of the lower eyelid may look similar in the mirror, but they develop for very different reasons.

And things are often even more complicated than that. In a study of 65 people with dark circles, 78% had a mixed type, meaning that two or even three different factors were present at the same time.

So if you have three eye creams sitting in a drawer that never seemed to make much difference, there may be a simple explanation: they may have been targeting a different cause from yours.

Why does the under-eye area show darkness so easily?

Very little can hide beneath the skin around the eyes.

Lower-eyelid skin is among the thinnest on the body, measuring only a fraction of a millimetre in some areas. Directly beneath it lie blood vessels, muscle, fat and bone. Changes that would barely be noticeable elsewhere on the face can become obvious here.

Because the skin is so thin, the underlying vascular network can show through as blue, purple or grey. A deeper hollow creates a shadow. Melanin produces genuine brown pigmentation, while a bag beneath the eye does the opposite of a hollow: it projects forward and casts a shadow below it.

Light therefore makes a surprisingly large difference. The same under-eye circles can appear much darker beneath harsh overhead office lighting and considerably softer in front of a window.

Your face has not changed in ten seconds.

The angle of the light has.

1. When the problem really is pigmentation

Some people remember having dark circles from a very young age.

They did not appear after a stressful period or suddenly in their thirties. They were simply always there, usually with a brown or brown-grey colour. And quite often, if you look at an old photograph of your mother at the same age, you will recognise the same eyes.

This is not simply an impression. In a study of 200 people with periorbital hyperpigmentation, 63% reported a family history. Atopy was present in 33%, while 32.5% reported frequently rubbing their eyes.

That last point deserves particular attention because it is one of the factors we can actually influence and one of the easiest to overlook. An allergy, a little itching, watery eyes, and your hand instinctively reaches up again and again without you even realising it.

Repeated rubbing continually irritates the delicate skin around the eyes. Over time, that irritation can stimulate increased melanin production, gradually making the area appear darker. This is known as post-inflammatory hyperpigmentation.

Sun exposure adds another factor for a very simple reason. Sunscreen goes on the forehead, cheeks and nose, but many people stop short of applying it close to the eyes. As a result, one of the thinnest and most sensitive areas of facial skin can remain more exposed to ultraviolet radiation.

When pigmentation is the main issue, adding volume is not the answer. The focus shifts to the colour itself. Skin whitening mesotherapy can form part of a treatment plan for hyperpigmentation, alongside daily sun protection and control of the factors that keep the area irritated.

And that final point matters. If the irritation continues, the pigmentation has every opportunity to return.

2. When the blue and purple tones come from beneath the skin

Vascular dark circles have a completely different character.

Instead of brown, they tend to appear blue, purple or grey, and one of their defining features is that they do not necessarily look the same every day.

There are mornings when you barely notice them and others when your eyes suddenly look far more tired. A poor night’s sleep, crying, an allergic flare-up or temporary puffiness can all make them more visible.

The reason lies beneath the surface.

The vascular network sits extremely close to the skin. As the skin becomes thinner, the vessels beneath it become easier to see. With age, gradual loss of collagen and skin density can make this effect even more noticeable.

In this case, focusing only on the colour misses part of the problem. Attention also turns to the quality of the skin itself.

Eye boosters are used to improve hydration and skin texture, while polynucleotides have attracted considerable interest in recent years.

In a recent multinational observational study, 61 people underwent three polynucleotide sessions at intervals of two to three weeks, with an improvement in the appearance of dark circles reported after treatment. An interesting finding was that good responses were also seen in cases with a structural component.

Findings such as these have added to the growing interest in treatments that focus not only on adding volume, but also on improving the quality of the existing tissue.

3. When there is no dark colour at all, only a shadow

This is perhaps the most deceptive category.

The skin itself can have an entirely normal colour, yet the eyes still appear permanently tired.

Often the reason is the tear trough, the hollow that extends from the inner corner of the eye towards the cheekbone.

In some people it is already prominent in their twenties. Nothing has aged prematurely. Their bone structure and the natural distribution of facial tissues are simply enough to create that contour.

Over time, however, the picture changes. The midface gradually loses volume and support, and the transition between the lower eyelid and the cheek becomes less smooth.

This is also where one of the most important changes in the way the area is assessed has taken place.

Today, the tear trough is not viewed as an isolated hollow that simply needs to be filled.

A 2024 expert framework published in the Aesthetic Surgery Journal emphasised evaluating the entire surrounding anatomy, including the cheekbones, midface support, fat distribution, bone structure, skin quality and the presence of under-eye bags.

In other words, the place where you see the shadow is not necessarily the place that needs to be treated.

In some faces, the shadow is partly created lower down, where the midface has lost support. Restoring volume around the cheekbones can soften the transition between the lower eyelid and the cheek, improving the appearance of the tear trough without necessarily treating the hollow directly.

Hyaluronic acid remains an extremely valuable option for appropriately selected patients.

What has changed is the thinking behind how it is used.

Seeing where the shadow falls is not enough.

You need to understand why it is there.

4. When the “dark circle” is actually an under-eye bag

There is also the exact opposite situation: instead of pigmentation or a hollow, the problem is projection.

When lower-eyelid fat protrudes forward, even slightly, overhead light can cast a shadow immediately beneath it. To the eye, that shadow looks like a dark circle even though the skin itself may be completely normal in colour.

The important distinction here is between temporary fluid retention and protrusion of the lower-eyelid fat.

Fluid retention changes throughout the day. You may wake up with heavy, swollen eyes and find that the area looks considerably better a few hours later. Salt, alcohol, sleep and general fluid retention can all influence it.

Fat protrusion behaves differently. The bag tends to remain relatively stable throughout the day.

And that distinction changes the treatment approach.

When an area is already projecting forward, adding more volume is not automatically the right answer and can sometimes make the lower eyelid look heavier rather than more rested.

Mild skin laxity can be addressed with treatments aimed at improving firmness and skin quality. When significant fat protrusion or excess skin is the dominant issue, surgical options such as blepharoplasty may also enter the discussion.

Not every under-eye area needs filler.

Knowing when it does not is just as important as knowing when it does.

Sometimes the explanation is not purely aesthetic

Dark circles are easy to treat as an exclusively cosmetic concern, but medical history can occasionally tell us more than the mirror.

Allergies are a good example. Congestion, puffiness and repeated rubbing can work together to make the under-eye area look darker.

Iron-deficiency anaemia has also been associated with periorbital hyperpigmentation in observational studies. In one Indian study, anaemia was found in half of the participants.

That does not mean dark circles diagnose anaemia, nor that everyone with them needs a blood test. But when the change appears suddenly, is accompanied by symptoms such as unusual fatigue, or there is a relevant medical history, further medical evaluation may be appropriate before focusing exclusively on cosmetic treatment.

Medication matters too.

Certain prostaglandin-analogue eye drops used in glaucoma treatment have been associated with loss of periorbital fat, deepening around the eyelids and changes in pigmentation. In a study of 105 patients, signs of prostaglandin-associated periorbitopathy were recorded in 93.3% of bimatoprost users, 70% of travoprost users and 41.4% of latanoprost users.

There is an interesting twist here: bimatoprost is also used in prescription treatments designed to promote eyelash growth.

Longer lashes can therefore coexist with changes in the tissues around the eye.

None of this can be discovered simply by looking at a dark circle.

Someone has to ask the right questions.

Is lack of sleep really to blame?

Yes and no.

A sleepless night absolutely shows on the face.

In an experiment published in Royal Society Open Science, 25 people were photographed after two nights of normal sleep and again after two nights restricted to approximately four and a half hours. A group of 122 observers, unaware of the sleep conditions, rated the sleep-deprived faces as less healthy, more tired and less attractive. They were also less inclined to socialise with them.

So sleep clearly affects how a face is perceived.

But that is very different from saying that poor sleep causes every type of dark circle.

In a separate study of 116 people, sleep quality was not significantly associated with the severity of periorbital hyperpigmentation. Factors such as family history, sun exposure and eye rubbing showed clearer associations.

Good sleep therefore helps the overall appearance of the face.

It does not change your bone structure, erase inherited pigmentation or move protruding fat back into place.

If only it were that simple.

And what about eye patches? Do they actually do anything?

Yes. Just not everything TikTok occasionally promises.

A cool eye patch before a night out or after a difficult night can hydrate the skin and temporarily reduce puffiness. The area can look fresher and the eyes more rested, and that is a perfectly real effect. It is also temporary.

A deep tear trough will not change shape in twenty minutes. Melanin does not disappear because something has been placed over the skin, and a bag caused by fat protrusion will still be in exactly the same place when the patch comes off.

Eye patches are not useless.

They simply have a different job from the one we sometimes expect them to do.

Hyaluronic acid under the eyes: when does it really make sense?

Hyaluronic acid can produce a very natural, rested appearance beneath the eyes, particularly when genuine volume loss and a pronounced tear trough are present.

The key is not to treat every shadow in exactly the same way.

One face may benefit from subtle correction of the tear trough. Another may need greater support through the cheekbones. If prominent bags or persistent puffiness are present, the approach changes again.

That is why the entire area needs to be assessed before treatment, rather than focusing only on the deepest point seen in the mirror.

Duration is another interesting part of the discussion. In a retrospective study of 155 patients, improvement remained visible at 18 months, suggesting that hyaluronic acid in this area may last longer than was traditionally assumed.

Ultimately, the best result does not come from adding more product.

It comes from using the right amount, in the right place, in the face that is actually likely to benefit from it.

So what is actually causing the darkness under your eyes?

A proper assessment begins by working out whether the darkness comes mainly from pigmentation, visible vessels, facial anatomy, puffiness or a combination of these factors.

Lighting matters. So do the tear trough, cheek support, skin quality and the presence of under-eye bags. Medical history also matters, including allergies, frequent eye rubbing and medications that can affect the area.

At Cosmetic Derma Medicine, treatment planning for the eye area is based on this overall assessment rather than on a single approach applied to everyone.

Because the four causes are not four different ways of describing the same problem.

They are different problems that can happen to look remarkably similar.

And once the cause becomes clear, choosing the right treatment becomes much more straightforward.

Applying concealer under the eyes can easily become one of those things you do without even thinking about it.
