You look in the mirror, point to a specific line beside the nose or mouth and say, “This is what bothers me.”

It is natural to expect the treatment to begin exactly there.

Yet the doctor may first examine your cheekbones, the area beneath your eyes, your temples, chin or jawline. You may even be asked to turn to the side or smile before the doctor decides where treatment should actually begin.

This does not mean that the wrinkle that brought you to the clinic is being ignored. It means that the doctor is trying to understand why it has become more noticeable.

The face does not age like a flat surface on which wrinkles simply appear one after another. Over time, changes occur in fat distribution, bone support, ligaments, soft tissues and the quality of the skin itself. These changes interact with one another, which is why the area we eventually notice in the mirror is not always where the underlying change began.

This is precisely where the philosophy of Liquid Lifting lies. The aim is not to fill every individual line, but to restore balance and support where needed without changing the natural character of the face.

What Do We Really Mean by Liquid Lifting?

The term Liquid Lifting refers to a non-surgical approach to facial rejuvenation using injectable treatments, primarily Hyaluronic Acid, planned according to the overall facial anatomy rather than focusing solely on an isolated wrinkle.

The word “lifting” can easily create the impression that the treatment simply involves using more filler to “lift” the face. In practice, the philosophy is quite different.

The aim may be to restore projection to a cheekbone that has lost some of its volume, balance the lower third of the face through careful enhancement of the chin, improve jawline definition or directly treat a line when local correction is genuinely indicated.

There is therefore no fixed set of areas that “must” be filled during every Liquid Lift treatment. The face first needs to be assessed as a whole.

A Wrinkle Can Be the Result of Several Different Changes

Nasolabial folds, the lines that extend from the sides of the nose towards the corners of the mouth, are a characteristic example.

In some people, they remain relatively subtle for many years. In others, they become more pronounced even when the skin does not show particularly deep wrinkles elsewhere.

Their appearance does not depend on a single factor. Soft-tissue anatomy, changes in midface volume, skin quality and elasticity, deeper facial structures, as well as continuous movement during speaking and smiling, can all play a role.

The same visible concern does not necessarily have the same underlying cause. In one person, loss of support in the midface may be more significant. In another, the line itself may require more direct local treatment, while in someone else skin quality may contribute so substantially that adding volume alone would not provide the most comprehensive solution.

Treatment therefore does not begin with the question, “Where is the deepest line?” It begins by asking what is making that line appear so prominent in this particular face.

Why Is the Midface So Important?

The cheekbones and cheeks are much more than areas where volume can simply be added.

Beneath the skin of the face are different superficial and deep fat compartments with specific anatomical relationships. These do not all change in the same way with age. Some may lose volume, others may change in distribution, while the underlying bony support also gradually evolves.

These changes can gradually alter the appearance of the cheeks. The cheekbones may appear less defined, the transition between the under-eye area and the cheek may change, and lines lower down the face may become more noticeable.

When there is a genuine indication, restoring selected areas of the midface can improve facial proportions without requiring excessive amounts of filler to be placed lower down.

This does not mean that every line around the mouth can be “corrected through the cheekbones”. Such a generalisation would be just as inaccurate as treating every patient in exactly the same way.

It means that before a treatment plan is decided, the entire anatomical context in which a particular change appears needs to be assessed.

Some Areas Influence the Overall Appearance Without Immediately Drawing Our Attention

The temples are a characteristic example.

Gradual volume loss in this area does not necessarily create an obvious wrinkle that immediately attracts attention. However, it can make the upper face appear more hollow, alter the transition towards the cheekbones and contribute to an overall tired appearance.

Something similar, although through different mechanisms, can occur in the lower face.

Chin projection and jawline definition contribute to the balance of the profile and to the proportions between the middle and lower thirds of the face. Sometimes what we perceive as a “heavier” lower face cannot be explained by a single fold, but rather by the way different facial structures relate to one another.

This is why assessment for Liquid Lift is not performed by looking at the face only from the front. The profile can reveal features that are not as apparent from a frontal view, while smiling shows how the tissues actually behave during movement.

Why Is It Important to Assess the Face in Motion?

Most before-and-after photographs are taken while the face is still. In everyday life, however, the face is constantly moving. We smile, speak and change expression, and the facial tissues move with us.

An area that appears balanced when the face is at rest may behave differently when the muscles become active. This is one of the reasons why the amount of filler used and the areas in which it is placed cannot be determined from a photograph alone.

Natural-looking results are not simply about whether a cheekbone looks attractive at rest or whether a wrinkle has become less visible. They also depend on whether the result remains natural and harmonious when the face moves, smiles, speaks and expresses emotion.

This becomes even more important when several treatments have already been performed. Excessive accumulation of filler in the midface can affect not only facial proportions but also how natural the face appears when smiling or moving.

Myth: “The Deeper the Wrinkle, the More Filler It Needs”

A pronounced wrinkle does not necessarily mean that a larger amount of hyaluronic acid is required. Before treatment, it is more important to understand what has made the line become more noticeable.

A line around the mouth may become more prominent because volume and support in the midface have changed over time. In such a case, addressing the area around the cheekbones may improve the overall appearance, meaning that the line itself ultimately requires less intervention.

In another face, however, the line may remain prominent even when the surrounding areas have good support. In that case, more direct application of hyaluronic acid to the area may be appropriate.

This is why two people with apparently similar wrinkles do not necessarily require the same treatment. The treatment plan is determined not only by how deep a line appears, but also by why it has developed or become more pronounced.

Myth: “Liquid Lifting Means Using Many Millilitres Throughout the Face”

It may, in fact, be quite the opposite.

The better we understand facial anatomy, the less sense it makes to add volume simply because there is a shadow, a fold or an area that has changed. Careful restoration of facial proportions may require smaller amounts in selected areas and no filler at all in others.

This matters for another reason as well. In recent years, increasing attention has been given to what is known as facial overfilled syndrome, a term used to describe changes that may occur when repeated filler treatments gradually accumulate and begin to affect the natural proportions or mobility of the face.

It is not simply a matter of “too much filler”. The total amount, the location and depth of placement, each person’s individual anatomy, the way the tissues move and, of course, any material that may already have been placed in the past can all play a role.

And this raises an interesting question: what has happened to filler that was injected one or two years ago?

Previous Filler Should Not Always Be Considered “Gone”

We often assume that the duration of the visible result is the same as the length of time hyaluronic acid remains within the tissues. The two, however, are not necessarily the same.

A 2024 study used magnetic resonance imaging to examine 33 patients who had received hyaluronic acid fillers in the midface. Residual filler was detected in all participants at least two years after treatment, while in some cases filler remained detectable several years after the most recent procedure. The researchers also emphasised that larger studies are needed to better understand the longevity of different products and how they behave in the tissues over time.

This does not mean that every filler remains for years or that it continues to provide the same aesthetic effect throughout that period. It does, however, highlight something important when planning a new treatment: the face does not necessarily start from a blank slate each time.

For this reason, before designing a new treatment plan, it is important to know when previous treatments were performed, which areas were treated, how much product was used and whether adequate volume is already present in the area.

In selected cases, when clinical examination alone does not provide sufficient information, ultrasound may also help identify previous fillers and allow for a more accurate assessment of the area.

Sometimes the Wrinkle That Bothers You Does Not Need More Volume

Not every facial change is caused by volume loss.

A fine superficial wrinkle may be more closely related to skin quality. Lines on the forehead or between the eyebrows are strongly influenced by muscle activity, while skin laxity may require a different approach from an area affected by genuine volume loss.

This means that Liquid Lifting may form only one part of a broader treatment plan.

Depending on the predominant concern, it may be combined with Botox for dynamic wrinkles, Skin Boosters or other treatments that focus more specifically on skin quality, as well as different technologies when tightening is the primary goal.

The right treatment is not determined by how many different tools are available. It comes from understanding which of them genuinely addresses the individual need in front of us.

Why Can It Sometimes Make Sense Not to Treat Everything on the Same Day?

The desire to complete a treatment in a single appointment is entirely understandable. However, there is not always a reason to treat every area at the same time.

When a treatment plan involves more than one area of the face, a gradual approach can help the doctor assess the effect of each step more clearly.

When lost volume in the cheekbone or cheek area is restored first, the overall appearance of the face may change. Once the initial swelling has subsided, it becomes easier to determine which areas still require treatment and which have already improved sufficiently.

This does not mean that Liquid Lifting must always be performed over several appointments. It simply means that the treatment plan can be adapted to the actual appearance and response of the face, without performing more intervention than is necessary.

The right amount is the amount that improves facial proportions while keeping the result natural and balanced.

What Should the Doctor Ultimately Assess?

Proper planning for Liquid Lifting begins before the treatment itself.

Facial proportions need to be assessed from different angles, including midface projection, the temples, the under-eye area, the chin and the jawline. The way the tissues behave during movement should also be observed in order to determine which changes are primarily associated with volume loss, which are influenced by muscle activity and which are more closely related to skin quality.

A history of previous aesthetic treatments is equally important.

And after all of this comes the personal element, which should never become lost in the anatomy: what is it that bothers you when you look in the mirror?

The doctor may identify several different anatomical features. That does not mean that all of them need to be corrected. The treatment plan should bring together facial anatomy with the genuine needs and wishes of the person being treated.

A Natural Result Does Not Mean a Face Without Lines

The goal is not to eliminate every line or alter the natural proportions of the face.

Some people may benefit from direct correction of a particular area. For others, restoring support or balance around it may be more important first, while there are also cases in which the feature that bothers someone in the mirror does not require filler at all.

At Cosmetic Derma Medicine, every Liquid Lifting treatment is planned according to an individual assessment of facial anatomy, proportions, tissue mobility and previous treatment history.

Because the line you point to in the mirror matters.
