Surgical Procedure

The aesthetic problem that rhinoplasty (nose plastic surgery) is called upon to address as an aesthetic and functional procedure, can vary, e.g. large (nose pyramid hypertrophy) or bent nose, crooked nose, large flaps, etc.

Apart from the aesthetic problems, rhinoplasty (nose job) helps in many cases to improve breathing, as it is very common for a nose aesthetic problem to coexist with scoliosis of the nasal septum, which often causes difficulty in breathing. Scoliosis of the nasal septum may be congenital or due to an injury.

Applications

Rhinoplasty is usually performed under general anesthesia and less often under local anesthesia or intoxication and the procedure takes about an hour. There are two surgical techniques, closed rhinoplasty and open rhinoplasty.
Both types of surgery are combined with septoplasty if there is also a functional problem or difficulty in breathing.

Closed Rhinoplasty

In closed rhinoplasty, no external incisions are made and access is through the nasal passages. The skin is detached upwards and the mucosa downwards so that the fine instruments can find a passage to the bony and cartilaginous skeleton of the nose. With special instruments (osteotome) and specialized techniques, the skeleton of the nose is reshaped and given the decided shape. A special splint is placed externally which will remain in place for about eight days and helps to maintain the shape of the nose, and gauze is placed inside the nose for 2 days. The final shape of the nose is completed after 4-6 months.

Open Rhinoplasty

In open rhinoplasty the basic stages are the same, but a small incision is made in the styloid, i.e. at the point where the two nostrils meet. It is an easier technique for the surgeon, as he has a better view and visibility of the anatomical abnormalities of the nose and can intervene more easily. The result is also more predictable. It has the disadvantage of a small scar, but this is almost invisible when done correctly and by an experienced plastic surgeon. In this case too, a special splint is placed for 8 days and the final result is visible after 4-6 months.

Postoperative Course

Immediately after the rhinoplasty, a special splint is applied to the nose and gauze is applied inside the nose, which is removed in about 2 days. Any bruising and swelling around the eyes are a common condition which subsides in the following days. The use of cold patches helps to reduce swelling and ecchymosis.

The pain is usually bearable in the nose area and the patient may experience a headache. In both cases the pain is relieved with painkillers, but the use of aspirin is not permitted.

The external splint is removed 7-8 days after surgery and care is needed in the following weeks to avoid injury to the nose.

Ten days to two weeks after the operation the nose appears completely normal and any bruising that has occurred has subsided and the patient can return to daily activities and work. The final result is seen up to six months after the procedure.
